I had the same problem. Clearing the cache didn't work. But I managed to get it working again...for how long remains to be seen.
This is what I did:
Clear cache and remove the GTA install files on your
Xbox. I left my save files so that I wouldn't have to start over.
Insert Disc 1 (the install disc) and complete the installation. Once you get the prompt asking you to insert Disc 2 (play disc) DO NOT INSERT THE DISC. Instead, hit the
Xbox home button and press Y to return to the dashboard.
THEN, when you're out of the game and in the dashboard, swap discs, and you should be able to play again without the errors.

I can tell you what's wrong because I work for GameStop and me and another customer had the same issue. After clearing my install, re-seating the hard drive, clearing my cache multiple times, turning off cloud saving, puting my save on a different storage device, etc. all of that DID NOT work! Your issue is that you have a dent more than likely in the hub divide (the plastic ring in the middle of your disk). Look around that in the light and you will probably see a spot that looks like someone jabbed it with a pin or something. I noticed it before I put the disk in the first time and thought "It should work." Well it doesn't. When that customer came in and did a defective return I looked at his disk and sure enough it had a dent just like mine. We opened up another copy to see if it had that dent and it didn't. I told him "I bet that is the problem, and when you try the new disk please call me and let me know if it fixed it." He called me an hour later and said "It fixed the problem!"
My advice to everyone with this issue is to go back to where you got it and do a defective exchange and make sure you don't see this dent. This will fix this issue I swear! I'm surprised no one has figured this out by now. I guess Rockstar had a batch of bad disks!
Resumindo: se não funcionar o que a Rockstar diz, e não funcionar jogar teu save na núvem, e não funcionar instalar o disco 1, ir pra home e depois inserir o disco 2, só te resta trocar o jogo....
